Home Visits

Our Physiotherapist will visit you in the comfort of your own home, or care home, to support you with the management of your condition in an environment that is most suited to you.

We can support you by providing education, advice, treatment and rehabilitation, where appropriate, for a vast range of conditions including:

  • Brain injury
  • Spinal cord injury
  • Stroke
  • Long-term degenerative conditions (for example: Parkinson's Disease, Huntington's Disease, Multiple Sclerosis)
  • Functional Neurological Disorder
  • Post-operative rehabilitation (for example, hip or knee replacement surgery, fracture)
  • Hospital admission recovery (returning home after an illness or prolonged hospital stay)
  • Mobility issues and falls prevention

Your sessions will start with an assessment and from there we will discuss your goals and work together to formulate a treatment plan.

We can liaise with family members, care providers and other medical professionals involved in your care. If you live in a care home, our Physiotherapist will also liaise with care staff and contribute to care plans as necessary to ensure your care is managed in the best way for you.

Care Home Services

We provide services to nursing, care and residential homes directly to support care providers in delivering holistic rehabilitative services for their residents.

There are increasing pressures within social care to enhance care delivery and, more so than ever, rehabilitation in care homes and community settings.

The support of an experienced Physiotherapist can provide the guidance needed to support residents and staff alike to improve resident wellbeing, engagement and delivering outcomes.

This provision of support would be bespoke to your service and could include:

  • Group exercise classes
  • Individual exercise programme prescription
  • Falls assessments
  • Mobility assessment
  • Care plan reviews
  • Rehabilitation / therapy goal setting
  • Advice and support for recovery following hospital admission

Contact us to discuss your needs and create a tailored package to meet the demands of your service.
